Medical Director
Born and raised in Montgomery County, Dr. Graf attended Albright College where he did his undergraduate training. He then went on to Lake Erie College of Osteopathic Medicine-Bradenton. Afterwards, Dr. Graf completed his Residency at Suburban Community Hospital. He participates in all aspects of geriatrics and internal medicine including hospital rounds, office hours, nursing home care and home visits. Dr. Graf is Board Certified in Internal Medicine and his clinical interests are dementia care and short term rehab.
Attending Physician
Dr. Alison J. Leff is an internist in Bryn Mawr, Pennsylvania. She received her medical degree from Philadelphia College of Osteopathic Medicine and has been in practice for more than 20 years.
Physiatrist
Dr. Robert J. Kreb is a physiatrist in Ardmore, Pennsylvania and is affiliated with multiple hospitals in the area. He received his medical degree from Perelman School of Medicine at the University of Pennsylvania and has been in practice for more than 20 years.
Attending Physician
Dr. Feltner specializes and is board certified in Internal Medicine. She is a native to the tri-state area and completed her residency outside of Philadelphia where, she also served as chief resident. Dr. Feltner most recently became a Fellow of the American College of Osteopathic Internists. She participates in multiple facets of internal medicine, including, but not limited to hospital medicine, critical care, geriatrics, primary care and aesthetic medicine. Dr. Feltner also has a Ph.D in behavioral neuroscience and completed her post-doctoral fellowship at the Center for Neural Science at NYU. She was also a visiting scientist to the prestigious Max Plank Institute of Biological Cybernetics in Tubingen Germany. Moreover, Dr. Feltner has taught at several universities in the area including NYU, CUNY, Gwynedd Mercy, Temple, and Jefferson. She enjoys all aspects of internal medicine and is passionate about patient-centered care. Dr. Feltner truly takes on an integrative approach to medical management. Dr. Feltner looks forward to developing life-long relationships with her patients to help optimize their health and help them achieve their personal wellness goals through all of life’s changes. Dr. Feltner was named “Top Doc on the Main Line” in 2020 and “Women in Medicine Top Hospitalist” in 2021.

Dr. Vinod K. Kataria is a geriatrician in West Chester, Pennsylvania and is affiliated with Chester County Hospital. He received his medical degree from Nalanda Medical College and has been in practice for more than 20 years.
Nephrologist
Dr. Michael R. Lattanzio is a nephrologist in Exton, Pennsylvania and is affiliated with multiple hospitals in the area, including Crozer Chester Medical Center and Chester County Hospital. He received his medical degree from Philadelphia College of Osteopathic Medicine and has been awarded Philadelphia Magazine Top Doc- 2023.

Crystal Richardson-Tokarczyk, RRT,BS is an experienced respiratory therapist, offering a 26 year background history in, assisting in diagnosing, treating and managing patients with complex pulmonary disorders. Reputable and friendly, Crystal is an ACLS certified, which is paired with excellent time management and critical thinking skills. She has worked in all facets of healthcare beginning with ER , lower level acute care as well as LTACH’s, pulmonary and cardiac monitoring rehabs. Also Crystal is proficient in maintaining respiratory equipment and determining the need for respiratory treatments/modalities.
